New Patient Registration

To register at Redwood House Surgery you will need to complete the patient registration forms only available from the surgery (at the moment). This is so we can gather the immediate medical information about you. We also offer a patient information pack about our surgery, please ask for one when you register.  You must be a patient living within our designated NHS boundary, please ring the surgery to check your address if you are unsure.

You may be required to have a New Patient Health Check with one of our Practice Nurses after registration at Redwood House. An appointment will be offered to suit you but if you need to see a GP before your Health Check this can be arranged.

We will need to know the immunisation status of children under 6 years of age.

New patients can express a preference as to which GP they would like to be registered with and receive services from.
